WIC is part of the Nursing Division of Clermont County Public Health.  We are located at 2400 Clermont Center Drive, Suite 200, Batavia, OH 45103.

WIC is open Monday through Friday 8:30 a.m. – 4:30 p.m. We offer some late appointments on Mondays.

What to bring to your certification or re-certification appointment:

• WNC (WIC Nutrition Card)
• Child (Bring every child who has an appointment)
• Identification for the parent/guardian, which may include a driver’s license, social security card, or medical card
• Identification for woman and child participant, which may include medical card shot records or birth certificate
• Proof of income evidenced by Medicaid or SNAP card or the last three pay stubs plus any other household income i.e., SSI, child support, savings, OWF, or unemployment
• Shot records
• Proof of pregnancy
• Proof of address, which may include driver’s license or piece of mail
• Updated formula prescription may be needed

What to bring to your nutrition education appointment:

• WNC (WIC Nutrition Card)
• Identification such as a driver’s license, social security card, or medical card
• You may need to bring your child and/or an updated formula prescription.
